This is a per diem position.

Responsibilities:
1. Conducts and documents the nursing assessment of the health of the patient by collecting objective and subjective data, as outlined by facility policy, obtained by examination, interviews, and written records in the timeframe based on facility policy. This position then sorts, selects, reports and records the data, then validates, refines and modifies the information through interaction with the patient/resident, family and health team members.
2. Based on information collected during the nursing assessment establishes and documents nursing diagnoses, which serves as the basis for the plan of care.
3. Develops and modifies the plan of care based on the nursing assessment and nursing diagnosis. This includes identifying priorities in the plan of care, setting realistic, measurable goals, identifying nursing interventions and identifying actions to maintain comfort, support human functions and responses, to maintain an environment conducive to well being and provide health teaching and counseling.
4. Implements the plan of care by initiating nursing interventions by providing direct care, assisting with care and delegating care. This includes providing an environment conducive to safety and health, documentation of interventions and responses to care, and communicating nursing interventions and observations of responses to care to other health team members and supervising the process of obtaining and implementing the physician orders.
5. Evaluates the responses of patients/residents to nursing interventions. This evaluation may involve the patient/resident, family and other health team members. Shall document in the medical record and communicate to appropriate health team members the information collected. Will use the information to reassess the health status, modify the nursing diagnosis, revise the plan of care and determine changes in nursing interventions.& nbsp;

Job Summary:
Must hold a valid Wyoming license for a Registered Professional Nurse. Must possess a BLS certification. ACLS, PALS, and NBR are preferred and are required with-in 1 year of employment.
Effective human relation and communication skills are necessary.
